Antidepressant Medication Review Form

Your antidepressant medication is now due for review.

Please read the following and indicate your response below before returning this form before your next prescription is due.

Antidepressant medication is used to treat low mood and anxiety symptoms and is usually intended to be used for a course of approximately 6 months. Whilst some people benefit from longer or ongoing medication, most people find they are able to reduce and stop their medication after 6 months, and they will usually find the beneficial effect obtained from the course of treatment will be maintained even after stopping. People who continue to take antidepressant medication should ideally have this reviewed approximately every 6 months.

If your symptoms are improved to a satisfactory level and are stable, you should consider reducing and stopping your medication. This should be done gradually usually over a number of weeks and your GP will advise you how to do this. Please note: if your medication is reduced and your symptoms return, then your medication could be increased again.

If you have ongoing troublesome symptoms and do not feel able to reduce your medication at this time, please let us know by indicating this below. In this case, you may also wish to book an appointment for review with one our Practice Mental Health Practitioners or a GP.
