Travel Information

If you require any vaccinations relating to foreign travel you should complete the Travel Questionnaire  which is available below or from reception and make an appointment for the travel clinic. You should be sure to complete the full details of your travel itinery including  countries and areas within countries that you intend to visiting.

It is important to complete the travel questionnaire as early as possible  - at very least 6 weeks before you travel so that an appointment with the travel clinic can be arranged. At the travel clinic the nurse will arrange for a prescription to be left for you for vaccines if needed and you will be given a further appointment to be given your vaccines. Please note that some vaccination programmes require 6 months to fully complete.

Some vaccines and all antimalarial medication are only available on a private prescription and these incur a charge over and above the normal prescription charge.  This is because travel vaccination is not included in the services provided by the NHS.

To help us offer the appropriate advice, please complete the form below, and send it to us.  We will then contact you regarding your vaccinations and to arrange an appointment at our travel clinic.
